Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A code compliance inspection was initiated on 03/19/2021 and concluded on 03/19/2021. The director was contacted virtually to initiate the inspection. There were 44 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #:	22VAC40-191-40-D-2
Description:	Based on staff record review, the center did not ensure one of five staff records had documentation of a completed Sworn Statement or Affirmation prior to employment. Evidence: 1. The record of staff #2 (start date: 3/25/19) had documentation of a completed sworn statement of affirmation dated 4/2019.
Plan of Correction:	I will not let staff start until have all background checks.
Standard #:	63.2(17)-1716-B-3
Description:	Based on record review and an interview, the center did not ensure to have at least one person on staff that has been trained to conduct daily simple health screenings of children. Evidence: During the inspection of five staff records and interview with the center representative, it was discovered there were no staff that received training in daily health screenings. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings.
Plan of Correction:	I will work on daily health training now and will have it done by the weekend.

Description:	Based on staff record review, the center did not ensure two of five staff records had documentation of fingerprint based national criminal history search results prior to employment. Evidence: 1. The record of staff #1 (start date: 1/27/20) had documentation of fingerprint based national criminal history search results dated 1/29/20. 2. The record of staff #5 (start date: 3/8/21) had documentation of fingerprint based national criminal history search results dated 3/18/21.

Description:	Based on record review and an interview, the center did not ensure one of five staff records contained documentation of sex offender registry check results from any state in which the individual has resided in the past five years. Evidence: 1. The record of staff #5 (start date: 3/8/2021) did not have documentation of sex offender registry check results from any state in which the individual has resided in the past five years. 2. The Sworn Statement or Affirmation for staff #5 indicated that the staff has lived in one other state within the past five years. 3. The center representative stated that sex offender registry check results were not obtained from the perspective state.
Plan of Correction:	I will work on staff #5's background checks now.
