Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A code compliance inspection was initiated on April 19, 2021 and concluded on April 20, 2021. The director was contacted by telephone to initiate the inspection. The center is physically closed but is providing virtual services. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on record review, in two of two staff records reviewed, the center did not ensure that staff have results of a Search of central registry within 30 days of employment or commencement of service.

Evidence: The director confirmed that there were not results of a search of the Central Registry for Staff #1 and Staff #2; both staff members have been employed at the center for more than 30 days.

Plan of Correction: Director will send the form to each staff person to have them complete it, send proof to the Licensing Inspector and then mail it to the Office of Backgound Investigations. The results will then be forwarded to the Licensing Inspector.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review, in one of two staff records reviewed, the center did not obtain a copy of the results of a sex offender registry check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence: The director confirmed that there was no documentation of out of state sex offender or central registry results for Staff #1 who indicated on the sworn statement as to living outside of the state of Virginia within the last five years.

Plan of Correction: Director will contact the state of Ohio and submit any forms to said employee to submit for processing. Proof of sending the documents to Ohio as well as proof of results will be sent to the licensing inspector once received.
