Comments:
This subsidy inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic. Preliminary data was obtained from the vendor on May 10, 2021. An initial subsidy inspection was initiated on May 13, 2021, at which time the vendor was contacted by telephone to initiate the inspection, and concluded on May 14, 2021. The inspector emailed the vendor a list of items required to complete the inspection. The vendor reported there were seven children in care. The inspector reviewed two children?s records and two staff records submitted by the vendor to ensure compliance with the health and safety regulations. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Violations:
Standard #: 22VAC40-665-520-B-3
Description: Based on record review, in two of two children's records reviewed, the applicant did not ensure that each child's record was documented with all required parent information.

Evidence: The Director confirmed that one parent's employment information to include name, address, and telephone number had not been documented in the records for Child #1 and Child #2.

Plan of Correction: Director will have the parent to fill out the employment information.

Standard #: 22VAC40-665-770-B
Description: Based on a review of documentation, the applicant did not ensure that the emergency preparedness plan contained all required procedural components.

Evidence: The Director confirmed that the emergency preparedness plan did not contain the following components:

1. Evacuation Procedures did not include accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during evacuation or relocation.

2. Shelter-In-Place procedures did not include accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during shelter-in-place.

3. Lockdown Procedures and all required components.

4. Staff training requirements, drill frequency, and plan review and update.

5. Continuity of operations procedures to ensure that essential functions are maintained during an emergency.

Plan of Correction: Director will update and add all required components to the plan.
