This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review and interviews.
A subsidy health and safety inspection (SHSI) was initiated on August 10, 2021 and concluded on August 11, 2021. There were no children present. A code compliance inspection was also conducted on this date. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-665-580-H Description: Based on review and interview, vendor did not ensure staff attend at least 16 hours of training annually.
Evidence: The records for staff #1 and staff #2 did not have documentation of 16 hours of annual training. Person in-charge stated training was completed but not documented.Plan of Correction: A log-in sheet documenting training information will be used when staff attend facility trainings. Documentation of all trainings completed will be kept in each staff record.
