Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program.

A annual subsidy health and safety inspection (SHSI) was initiated on 09/29/2021 and concluded on 09/30/2021. There were 22 children present with 4 staff supervising. A code compliance inspection was also conducted on this date.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-665-540-B
Description: Based on record review, the center did not ensure one of two staff records contained documentation of subsequent a Tuberculosis screening which is required at least every two years from the date of the initial screening, or more frequently if recommended by a physician.
Evidence:
The record of staff #2 (start date: 1/1/1999) did not have documentation of a current tuberculosis screening.

Plan of Correction: The staff will get a copy of it within 10 days.

Standard #: 22VAC40-665-580-H
Description: Based on record review, the center did not ensure two of two staff records contained documentation showing that the staff completed the 16 hours of annual training including the Annual Virginia Health and Safety Update Training.
Evidence:
1. The record of staff #1 (start date 1/1/99) did not have documentation of the required 16 hours of annual training including the Annual Virginia Health and Safety Update Training which was last completed on 9/8/2020.
2. The record of staff #2 (start date 1/1/99) did not have documentation of the required 16 hours of annual training including the Annual Virginia Health and Safety Update Training which was last completed on 9/5/2020.

Plan of Correction: They will complete [Annual VA Health and Safety Update Training] by next Monday. They will complete [annual training] by November 1st.

Standard #: 22VAC40-665-580-K
Description: Based on record review and an interview, the center did not ensure to have at least one person on staff that has been trained to conduct daily simple health screenings of children.
Evidence:
During the inspection of two staff records and an interview with the center representative, it was discovered there were no staff that received training in daily health screenings within the past three years. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings. Daily health screenings should be completed every three years. The record of staff 1# and #2 had documentation of a Daily Health screening certificated dated 6/7/2017.

Plan of Correction: All the staff had Daily Health Observation Training and had new training scheduled for May 2020 but they stated due to the pandemic it was canceled. They are taking the course on line this weekend.
