Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program.

A code compliance inspection was initiated on 09/29/2021 and concluded on 09/29/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 22 children present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and and supervision. A total of 3 children?s records, a center representative record and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on record review, the center did not ensure that the center representative had documentation of central registry results and a completed sworn statement or affirmation upon receipt of religious exemption status.
Evidence:
The record of the center representative (start date: 8/12/15) did not have documentation of a completed sworn statement or affirmation and central registry results.

Plan of Correction: We will let the designated staff handle the children and staff records as they do for all of the other daily operations and inspections. Our directors and their assistants are hands on in the center with the children the reason I was asked to volunteer to handle inspections only.

Standard #: 63.2(17)-1716-B-3
Description: Based on record review and an interview, the center did not ensure to have at least one person on staff that has been trained to conduct daily simple health screenings of children.
Evidence:
During the inspection of two staff records and an interview with the center representative, it was discovered there were no staff that received training in daily health screenings within the past three years. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings. Daily health screenings should be completed every three years. The record of staff 1# and #2 had documentation of a Daily Health screening certificated dated 6/7/2017.

Plan of Correction: All the staff had Daily Health Observation Training and had new training scheduled for May 2020 but they stated due to the pandemic it was canceled. They are taking the course on line this weekend.

Evidence:
The record of the center representative (start date: 8/12/15) did not have documentation of fingerprint based national criminal history search results.
