Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program. A code compliance inspection was initiated on 10/20/21 and concluded on 10/28/21. The director/in-charge person was contacted by telephone and a virtual inspection was conducted. There were 33 children present, ranging in ages from 2 to 5, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of 3 children?s records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable code sections and violations were documented on the violation notice issued to the program.

Standard #:	22VAC40-191-40-D-2
Description:	Based on record review and interview, the center failed to have staff sign a sworn disclosure statement prior to the first day of employment and have a completed central registry record check by the end of the 30th day of employment. Evidence: 1. Staff 1's date of hire was 9/1/20. The central registry record check results were dated 7/2/21 and the sworn disclosure was dated 5/19/21. 2. Staff 3's date of hire was 9/7/21. Per the record the central registry record check was mailed 10/12/21. 3. The center does not have the results for a central registry record check as of the date of this inspection per the director.
Plan of Correction:	In the future the director will ensure new staff will complete a sworn statement upon application and the paperwork for central registry will be completed and mailed upon hire.
Standard #:	63.2(17)-1716-A-4
Description:	Based on record review and interview, the center failed to ensure all staff have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him/her from caring for children under his/her supervision prior to start date. Evidence: Staff 3's start date was 9/7/21. The staff health report is dated 10/19/21
Plan of Correction:	In the future the director will ensure staff health reports are obtained prior to the first date of employment.

Description:	Based on record review and interview, the center failed to obtain fingerprint-based criminal history check determination letters prior to the first day of employment. Evidence: Staff 3' date of hire was 9/7/21. The fingerprint-based criminal history check determination letter is dated 10/27/21.
Plan of Correction:	In the future the director will ensure no staff will be hired until the fingerprint-based criminal history check determination letter is obtained.
