The sworn statement for staff 2 documents living in Ohio and California within the past five years prior to the first day of employment. The staff record does not have documentation of completing sex offender registry searches in Ohio and California.Plan of Correction: Sex offender registry searches will be completed for staff 3. In the future the administration will ensure out of state sex offender searches will be completed as required.
