Comments:
The licensing inspector for Charlottesville Catholic School conducted a complaint inspection in response to a complaint that was received on 11/18/21. Interviews with administration and involved parties were conducted relating to allegations of lack of supervision and areas accessible to children were not free from hazards. The information gathered during the investigation supports the allegation, so the complaint is determined to be "valid." There were violations resulting from this complaint investigation and are identified on the violation notice. If you have any questions, contact your licensing inspector at (540) 848-4123.

Complaint related: Yes
Description: Based on a review of the staff handbook and interview, the center failed to implement the center's procedures for appropriate supervision of all children in care.

Evidence:

1. Per the current parent handbook under the section labeled Playground Safety "Children shall be supervised at all times on the playground...." "The staff shall be positioned so that all areas of the playground that the children use are visible at all times."
2. Per interview with Adult 1, on 10/18/21 her child, child 1, was playing by the baseball field and tripped on a metal rake. The tines on the rake went into child 1's knee. Her other child, child 2, went to get help when no staff came to help child 1. The school nurse contacted adult 1 as she just so happened to still be at the school the day of the incident. The nurse helped to get the bleeding under control and recommended medical treatment. The pediatrician sent them to the ER. The rake punctured the knee resulting in surgery to clean the knee joint.
3. Per the director, there were 32 children in care at the time of arrival, but many had left by the time the incident occurred. There were two staff supervising the remaining children. The children were outside playing at the time.
4. The administrator stated the outdoor play space used by the children consist of the playground, sidewalk, blacktop with basketball court, soccer field, and baseball field.

Plan of Correction: The outdoor play space has been reduced to now include the mulched area, sidewalk and blacktop.
The Office of Risk Management of Richmond met to assess the grounds to help implement follow up on 12/16/21.

Description: Based on a review of the staff handbook and interview, the center failed to establish and implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards.

Evidence:

Per the administrator the school does not have a procedure to check the outdoor space for obvious injury hazards. The baseball field is used by a third party and a rake was left on the field by them that resulted in a child being injured.

Plan of Correction: The maintenance staff are now sweeping the grounds every morning after morning care line.
