Comments:
This inspection was conducted by licensing staff.

A complaint inspection was initiated on 7/7/2021 and concluded on 7/15/2021. The in-person component was completed on 7/15/2021. There were 26 children present, ranging in ages from one year to eight years , with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-665-800-C
Complaint related: No
Description: Based on observation the vendor failed to ensure that bottle fed infants do not use their bottle while in the child designated sleeping location.
Evidence:
1. On 7/15/2021 a male child was in a crib bed with a bottle in his hands.

Plan of Correction: All staff reminded that bottles are not to be placed in the cribs. All infants are to be held when feeding bottles.

Description: Based on observation the vendor failed to ensure that areas and equipment of the center are maintained in a safe and operable condition.
Evidence:
1. On 7/7/2021 it was observed that the mattress in the crib had a tear that was approximately 12 inches, with smaller tears throughout the mattress.
2. The top crib railing looked as if it has been shaved down so thin that it was breaking in half when touched.
3. The Little Tykes house in the toddler area has a broken top which is a pinch point.
4. In the infant area, above the sink the daylight can be seen through the crack in the wall.
5. The boys bathroom had baseboards on the floor with nails exposed and accessible to the children.
The baseboard in the girls bathroom was coming off the wall.

Plan of Correction: 1. Mattress was thrown away.
2. Crib was removed from the center that day.
3. The top was removed from the play house, later removed from the center.
4. Cracks and walls are being repaired.
5. The baseboards are being repaired or removed.

Description: Based on observations the vendor failed to ensure that all hazardous substances are kept in a locked place preventing access by children.
Evidence:
1. On 7/7/2021 in the school age area there was a bucket of cleaning supplies and the ledge surrounding the sound room area. This bucket had spray cleaner and sanitizing cleaners inside. This was accessible to the children. On a table was a can of Lysol spray. This table was being used by children in care.
2. On 7/15/2021 the storage room was open, inside was disinfectant spray, cleaning supplies including Clorox.

Plan of Correction: All hazardous substances have been removed from the classroom areas and will be kept in a locked place when not being used. The storage room door will be locked.

Description: Based on observations the vendor failed to ensure that medication is locked and not accessible by children.
Evidence:
1. On 7/15/2021 there was a bottle of Benadryl spray with a school age child.

Plan of Correction: All medication will be locked.
