Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. An unannounced code compliance inspection was initiated on 2/3/2022 and concluded on 2/14/2022. The person in-charge was contacted by telephone and a virtual inspection was conducted. There were 18 children present, ranging in ages from three to five, with three staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision and procedure. A total of two children?s records and two staff?s records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #:	22.1-289.031-B-3
Description:	Based on record review and interview, the center failed to establish and implement a procedure in which a daily health screening is conducted by a person trained to perform such a screening daily. Evidence: The administrator stated the center has a procedure to ensure a daily simple health check is conducted daily. However, no staff have received training.
Plan of Correction:	Staff that conduct daily simple health screening will be required to complete training.

Description:	Based on record review, the center failed to obtain fingerprint-based criminal history check determination letters prior to the first day of employment for one out of two staff records reviewed. Evidence; The record for staff 1 documents the hire date as 7/22/21. The fingerprint-based criminal history check determination letter is dated 8/2/21.
Plan of Correction:	In the future staff will not hired until the fingerprint-based criminal history check determination letter is received.

Description:	Based on record review and interview, the center failed to obtain out of state sex offender registry search and a search of child abuse and neglect registry or equivalent registry maintained by any other state in which a new staff has resided in the preceding five years for any founded complaint of child abuse or neglect against him/her prior to the first day of employment. Evidence: 1. The hire date for staff 2 is documented as 8/30/21. The sworn statement documents staff 2 lived out side the state of Virginia within the last five years. The record did not contain out of state sex offender registry search or a search of child abuse and neglect registry or equivalent registry maintained by any other state. 2. The administrator verified the center did not obtain out of state sex offender registry search or a search of child abuse and neglect registry or equivalent registry maintained by any other state for staff 2.
Plan of Correction:	In the future out of state sex offender registry search or a search of child abuse and neglect registry or equivalent registry maintained by any other state will be conducted for all staff prior to the first day of employment for all staff that have lived outside the state of Virginia within the preceding five years.
Standard #:	8VAC20-770-40-D-2
Description:	Based on record review and interview, the center failed to have staff sign a sworn disclosure statement prior to the first day of employment and have a completed central registry record check by the end of the 30th day of employment. Evidence: The record for staff 1 documents the hire date as 7/22/21. The results of the central registry record check is dated 10/8/21.
Plan of Correction:	In the future central registry record check will be mailed upon hire to ensure the results are received within 30 days. If the results are not received administration will follow up and document the file.
