This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program.
A self report inspection was initiated on 10/13/2021. The director was contacted by telephone , interviews and documents were reviewed and inspection was conducted.
The evidence gathered during the investigation supported the non-compliance with and violations were issued. Any violations during the course of the investigation can be found on the violation notice.

Standard #: 22.1-289.031-B-5 Description: Based on staff interview and documentation, the center did not ensure to keep all areas free of obvious injury hazards.
Evidence:
Staff#1 contacted the licensing inspector on October 13, 2021 of a incident that occurred on August 27, 2021. Staff #2 had a carabiner attached to a lunchbox with keys and pepper spray. Child #1 sprayed pepper spray in his face . Staff #1 and Staff #2 confirmed the pepper spray was not kept in a locked place. The center called 911 for medical assistance.Plan of Correction: All staff members personal belonging that contain lighter, medication or any hazardous material will be kept in a locked cabinet. Spoke to all staff immediately after incident happen individually on August 30, 2021 and August 31, 2021 and during a all staff meeting on September 14, 2021.
