Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program.
A code of compliance inspection was initiated on 04/08/2022 and concluded on 04/08/2022. The director was contacted by video call to initiate the inspection. There were 20 children present and 2 staff. The Inspector reviewed 2 staff records and 2 children?s records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.031-A
Description: Based on observation and an interview, the center did not ensure to post that the facility is exempt from licensure in a visible location.
Evidence:
The center did not post documentation informing the public and parents that the facility is exempt from licensure. The program's administrator stated that the information was not posted at the time of inspection.

Plan of Correction: We will have that posted by the end of the day.

Standard #: 8VAC20-770-40-D-2
Description: Based on record review, the provider did not ensure one of two staff records had documentation of central registry results within 30 days of employment and a sworn statement or affirmation completed prior to employment.
Evidence:
1. The record of staff #1 (start date: 9/02/21) had documentation of central registry results a completed 10/08/2021 and a sworn statement or affirmation dated 4/9/2022.

Plan of Correction: In the future, we will ensure background checks are completed within the required timeframe.
