Comments:
An unannounced code compliance inspection was initiated on 4/11/2022 and concluded on 4/11/2022 from 9:35 AM to 12:55 PM. There were 73 children present, ranging in ages from infant to five, with 15 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, procedures and medication. A total of three children?s records and three staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on record review and interview, the center failed to obtain a statement determining new staff are free of communicable tuberculosis (TB) at the time of employment and prior to coming into contact with children.

Evidence:

1. Per the director, staff 3's hire date was 3/7/22. There was no documentation of TB results in staff 3's file.
2. The director contacted staff 3. Staff 3 stated she had not obtained a TB yet due to difficulty with her doctor.

Plan of Correction: Staff 3 has been advised to obtain her TB screening as soon as possible. In the future no staff will be allowed to start working with children until TB documentation is received.

Standard #: 22VAC40-665-560-A
Description: Based on record review and interview, the center failed to obtain immunization records prior to the first date of attendance for two out of three children's records reviewed.

Evidence:

1. Child 2's first date of attendance, per the director, was 1/25/22. There was no immunization record in child 2's file.
2. Child 3's first date of attendance, per the director, was 12/1/21. There was no immunization record in child 3's file.
3. The director verified the center did not have immunization records for child 2 and child 3.

Plan of Correction: During the inspection child 2's immunization record was faxed to the center and provided to the inspector. Child 3's immunization record has been requested from the parents. In the future no child will be allowed to start until immunization records are received.

Standard #: 22VAC40-665-610-A
Description: Based on observation, the center failed to maintain all areas of the center in a clean, safe and operable condition.

Evidence:

The diapering pad in the changing room is torn exposing the absorbent sponge.

Plan of Correction: The changing pad will be replaced. All staff will be reminded to let administration know when areas of the center need to be repaired or replaced.

Standard #: 22VAC40-665-740-A
Description: Based on observation, the center failed to ensure children's hands were washed after diapering.

Evidence:

1. A teacher in the Fawn classroom was observed changing a two year old's diaper. The child did not wash their hands after diapering.
2. The director stated the center policy for handwashing after diaper is for children to wash their hands if they are capable of washing their own hands.

Plan of Correction: The center policy will be changed for all children's hands to be washed after diapering. All staff will be trained on the new policy.
