Today we discussed infant care.

Comments:
A complaint regarding unsafe feeding practices was received on January 18, 2022 and initiated on the same day. An onsite inspection was made at the facility on March 22, 2022. The investigation concluded on June 17, 2022. Telephone and onsite interviews were conducted. The information gathered supports the allegation and the complaint is found to be valid. Violations regarding obvious injury hazards were observed and appear on the violation notice.

Description: Based on observations and interviews the center failed to ensure that the premises accessible to children are free of obvious injury hazards.
Infant room:
1 - Infant 1 was left unattended with the bottle while laying on top of a horseshoe shaped pillow.
a. There was a loose fitting pillow case over the horseshoe shaped pillow that posed an entanglement hazard. Staff 1 stated that the pillow case was not the original case for the pillow.
2 - Plastic sandwich baggies were observed hanging above each infant crib. The baggies were attached to the wall with push pins. Push pins pose a potential choking hazard. Plastic baggies pose a suffocation hazard.
3 - Infant 2 was sleeping in a crib with a loose fitting sheet. A loose fitting sheet poses an entanglement hazard.
Physical plant
4- Additionally, a loosely installed plastic fencing was observed in the two-year-old room where a child in care was able to push the fencing over and landed on top of the barrier.
Infant feeding:
5 - Staff 1 stated verbally and in writing that the feeding schedule was which lead to infant 1 not being fed for a period of three hours.

Plan of Correction: 1 - All horseshoe shaped pillows have been removed from the program. New policy prohibits the use of these pillows in our program

2 - All baggies and push pins have been removed. New policy prohibits hanging any items over cribs. Except for appropriate toys and apparatus

3 - New mattresses have been obtained that allows for a tightly fitting sheet on the crib in question. New policy and staff training regarding appropriately fitting sheets to be checked prior to infant being placed in cribs. Staff have been instructed to inform management if there is any problem with loose fitting sheets.

4 - Barrier fencing has been removed. If a barrier is needed it will be install professionally and securely.

5 - A new policy requires enrolling parents to provide a written feeding schedule that is not alter unless and until the child matures and or developmentally requires a different feeding schedule which is required to be updated on the written feeding intake form.
