A complaint inspection was conducted on June 9, 2022 from 10:20am to approximately 12:45pm in response to a complaint regarding physical abuse of a child by staff. The complaint was received by the licensing office by March 9, 2022. Staff were interviewed, documentation was reviewed, and a tour of the facility was conducted. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program and were discussed during the exit interview.

Standard #: 8VAC20-790-710-1 Description: Based on observation and interview, it was determined that the center did not ensure that staff did not use physical punishment on children.
Evidence:
1. There is video footage of Staff #1 throwing a ball at Child #1 as punishment.
2. Staff #1 was immediately terminated.
3. Staff #2 confirmed that Staff #1 was terminated immediately.Plan of Correction: The center responded with the following: Staff #1 was immediately terminated from the center. There are all new staff members at the facility. The center will ensure that appropriate discipline training is provided to all new staff.
