A monitoring inspection was conducted on Monday, August 8, 2022 at the program. During the inspection, there were 20 children and 6 staff were present including administrative staff. The children were observed eating lunch, preparing for napping, and participating in various activities in designated classrooms . The sample size was 5 staff records.

Standard #: 22.1-289.031-A-6 Description: Based on review of documentation and interview, the provider did not ensure all written statements were provided to the parents or guardians of the children in the center.
Evidence: During the inspection on 08/09/22, there was no written documentation found for public liability insurance.
