8VAC20-790 Subsidy Regulations.

Comments:
A subsidy health and safety (SHSI) inspection was initiated on 8/1/2022 completed on 8/4/2022. A Code inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Violations are identified on the violation notice.

Violations:
Standard #: 8VAC20-790-670-H
Description: Based on observation the vendor failed to ensure that children less than 10 years of age shall always be within actual sight and sound supervision of staff.
Evidence:
1. Two LI's witnessed two staff standing in an empty room adjacent to the infant room. The infant room has four walls and a door that opens to the room were the staff where standing.
2. The five infants (ages 7 months-12 months) were alone and unsupervised, out of sight supervision of any staff for an undetermined amount of time.

Plan of Correction: The director reminded staff that children are never to be left alone. The staff are no longer at the center. Additional training is provided to all staff.

Standard #: 8VAC20-790-760-A-3
Description: Based on observation the vendor failed to ensure that staff wash their hands with soap and running water before and after changing a diaper.
Evidence:
1. During the inspection the toddler-2 year old's had been outside playing in the wading pools. The class came inside and began changing into dry clothes. Staff #3 & #4 changed approximately three soiled diapers on the carpeted floor.
2. Both staff #3 & #4 did not wash their hands before or after changing the soiled diapers.

Plan of Correction: The director reminded each staff that hands are to be washed, often and especially after diapering or toileting.
Additional training was provided.

Standard #: 8VAC20-790-760-B-3-c
Description: Based on observation the center failed to ensure that a nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing.
Evidence:
Following the toddlers-two year old's playtime in the wading pools, staff #3 & #4 changed three soiled diapers on the carpeted floor.

Plan of Correction: The director reminded staff that the changing tables upstairs at the classrooms are to be used when changing diapers.

Standard #: 8VAC20-790-760-B-6
Description: Based on observation the vendor failed to ensure that disposable diapers shall be disposed of in a leak-proof or plastic lined storage system that is either foot-operated or used in such a way that neither the staff member's hand not the soiled diaper touches an exterior surface of the storage system during disposal.
Evidence:
The toddler - two year old class came inside following playtime in the wading pools. Staff #3 & #4 changed diapers and pull-ups on the children. Three of the diapers were soiled. All of the diapers were disposed of in a trash bag that was on the floor, accessible to the children.

Plan of Correction: The director reminded staff that diapers are to be disposed in the foot pedal trash cans that are provided. Additional training was provided to staff.

Standard #: 8VAC20-790-770-B
Description: Based on observation and interviews the vendor failed to ensure that over the counter or nonprescription medication administered to a child must have written permission from the parent.
Evidence:
Written authorization to administer medication the child # 1 was not available and not maintained in the child's record.

Plan of Correction: The director reminded staff that the center has a no medication policy.
Additional training was provided to staff.

Standard #: 8VAC20-790-770-E
Description: Based on observation and interviews the vendor failed to keep a record of prescrtption and nonprescription medication given to children.
Evidence:
Child #1 was given an over the counter medication however, there was not record that included the name of the child to whom the medication was to be administered; amount and name of medication administered; the day and time the medication was administered; name of the staff administering the medication; any adverse reactions and any medication error.

Plan of Correction: The director stated that the center has a no medication policy.
If staff are MAT trained this policy may change and all the required paperwork will be completed.
Additional training was provided to the staff.

Standard #: 8VAC20-790-820-C
Description: Based on observation the vendor failed to ensure that bottle fed infants who cannot hold their own bottles shall be held when fed. Bottles shall not be propped or used while in the designated sleeping location.
Evidence:
During the inspection it was observed that an infant (4-5 months old) was in a crib on a boppy pillow with a bottle propped in his mouth.

Plan of Correction: The director talked with the staff and wrote a reprimand for each.
Additional training will be provided to all staff.
