A complaint inspection was conducted on 6/29/22 and 8/9/22 in response to a complaint that was received by the department on 5/31/22 regarding allegations in the care of the children at the center. Additional complaints were received during the investigation for the same allegations. Staff were interviewed, documentation was reviewed, classroom observations were completed, and a tour of the facility was conducted. Information gathered during the inspections supported the allegations so the complaint is deemed valid. The non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed during the exit interview.

Standard #: 22.1-289.031-A-3-c Complaint related: No Description: Based on observation and interviews, it was determined that the licensee did not ensure that for children, two years of age, a ratio of one staff member for every eight children, and one staff for every 16 children during rest period is maintained at all times children are in care.
Evidence:
1. There were 18 children present with staff #1 in the Preschool classroom during the rest period.
2. When staff #1 was asked how old the youngest child was, staff #2 stated that the youngest child was 2 years old. Based on the age of youngest child, the required staff-to child ratio would be one staff for every eight children, and one staff for every 16 children during rest period.
3. Staff #2 confirmed that the facility did not maintain the required staff-to-child ratio of one staff for every eight children, and one staff for every 16 children during rest period for this grouping of children.Plan of Correction: The facility responded: Corrected during the inspection as another staff finished her break and wen tot he classroom. We will ensure that all staff-to-child ratios are maintained at all times.
