8VAC20-790 Subsidy Program Vendor Requirements for Child Day Centers

A Subsidy Health and Safety inspection was conducted on August 30, 2022 to review supplemental health and safety requirements. This on-site visit was conducted from 11:10 am through 12:20 pm. Upon arrival, there were 14 children ranging in ages from 3 through 5 with 3 staff supervising. Four children records and seven staff records were reviewed and updated. Areas of noncompliance may be found on the violation notice.

Standard #: 8VAC20-790-560-A Description: Based on record review, the center failed to have two staff obtain tuberculosis screenings before beginning to work with children.
Evidence:
1. Staff #6 and #7 did not have documentation of obtaining a tuberculosis screening prior to working with children. Date of hire for Staff #6- 1/27/2022. Date of hire for Staff #7- 8/11/2022.
2. The director confirmed Staff #6 and #7 had not obtained a tuberculosis screening.Plan of Correction: Staff # 6 and #7 will immediately obtain a tuberculosis screening. Documentation will be sent to the licensing inspector.
Standard #: 8VAC20-790-560-B Description: Based on record review, the center failed to have one staff obtain an updated tuberculosis screening within 2 years of the prior screening.
Evidence:
1. Seven staff records were reviewed.
2. Staff #1 did not have documentation of obtaining an updated tuberculosis screening. Last screening was dated 11/07/2019.
3. The director confirmed Staff #1 had not obtained an updated tuberculosis screening.Plan of Correction: Staff #1 will obtain a tuberculosis screening as soon as possible. Documentation will be sent to the licensing inspector.
Standard #: 8VAC20-790-600-B Description: Based on record review the center failed to have one staff member complete required Virginia Preservice training within 90 days of employment.
Evidence:
1. Seven staff records were reviewed.
2. Staff #6 did not have documentation of obtaining required VA Preservice training within 90 days of hire. Date of hire was documented 01/27/2022.
3. The director confirmed Staff #6 had not taken VA Preservice training.Plan of Correction: Staff #6 will immediately obtain the VA Preservice training. Documentation will be sent to the licensing inspector.
