Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on October 11, 2022, from 12:15 p.m. through 1:45 p.m. At the time of arrival, there were 33 children in the care of 7 staff. Four children and five staff records were reviewed and update.

Standard #: 22.1-289.035-B-4 Description: Based on staff record review, the facility did not have documentation of obtaining an out of state central registry check for 1 staff member.
Evidence:
1. Five staff records were reviewed.
2. Staff #4 did not have documentation of an out-of-state central registry check.
3. Sworn statement signed by Staff #4 stated they had lived outside of the state of Virginia within the last 5 years.Plan of Correction: Out of state Central Registry search will be completed for Staff #4. Documentation will be sent to the licensing inspector as soon as received.
Standard #: 8VAC20-770-40-D-2 Description: Based on staff record review, the facility did not have documentation of obtaining a Virginia Central Registry search for 2 staff.
Evidence:
1. Five staff records were reviewed.
2. Staff # 3 and Staff #5 did not have documentation of obtaining a Virginia Central Registry search within 30 days of employment.
3. Staff #3's date of hire was documented 10/14/2021. Staff #5's date of hire was documented 07/28/2021.Plan of Correction: Virginia Central Registry searches will be completed for Staff #3 and Staff #5. Documentation will be sent to the licensing inspector as soon as received.
