Comments:
A monitoring inspection was initiated in person on October 11, 2022 and completed on October 24, 2022 at the program. During the inspection, there were 12 preschool children and 2 staff were present providing supervision. The children were observed participating in a lockdown assembly, transitioning to classroom, and working on manipulatives in their classroom. The inspector also inspected the bathroom, and other aftercare classrooms at the program. The sample size was 4 staff records.

Violations:
Standard #: 22.1-289.031-A-4
Description: Based on review of staff records and interview, the program did not ensure each person has been certified by a practicing or physician assistant to be free from any disability which would prevent from caring for children.

Evidence: In Staff#2's and Staff#4's records, there was no health report available for review during the inspection. Interview with staff confirmed the reports for both staff was not available for review during the inspection.

Description: Based on review of staff records and interview with staff, the program did not obtain an out of state central registry finding for staff as required.

Evidence: In Staff#3's record (date of hire: 02/17/22) there was not an out of state central registry findings found. Interview with staff confirmed the request for the central registry has not be completed and is not available during the inspection.

Description: Based on review of staff records and interview, the program did not ensure to obtain all central registry results within 30 days of employment.

Evidence: In Staff #2's record (date of hire 8/16/22) there was no central registry findings found. In Staff#3's record (date of hire: 2/17/22) there was no central registry findings found. In Staff#4's record (date of hire: 08/31/22) there was no central registry findings found.
Interview with staff confirmed the finding for each staff was not available for review during the inspection.
