Comments:
A code compliance inspection was conducted October 18, 2022. The licensing inspector was onsite from approximately 12:45pm until 2:00pm. There were 19 children present, ranging in ages from one to four years, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and four staff records were reviewed.


Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.031-A-9
Description: Based on observation, the center did not ensure compliance with all safe sleep guidelines recommended by the American Academy of Pediatrics.

Evidence:
1. Three infants were observed sleeping in playpens during the inspection.
2. The American Academy of Pediatrics states ?baby should sleep on a firm, flat non-inclined surface that, at a minimum, adheres to the June 2021 Consumer Product Safety Commission?s rule that any infant sleep product must meet existing federal safety standards for cribs. Parents should not use products for sleep that aren?t specifically marketed for sleep.

Plan of Correction: We will sleep the infants in cribs.

Standard #: 22.1-289.031-B-4
Description: Based on the review of five children's records, the center did not establish and implement procedures to ensuring that two children in the center are in compliance with the provisions regarding the immunization of children against certain diseases.
Evidence:
The record for Child #4 (first day of attendance 09/30/20 did not have documentation of immunizations. Staff #1 confirmed this information while the inspector was on site.

Plan of Correction: We have requested a copy of the child's immunization records.

Standard #: 22.1-289.058
Description: Based on observation and interview, the child day program, which was built prior to 2015, was not equipped with at least one carbon monoxide detector.
Evidence: A member of management stated that they have not yet installed a carbon monoxide detector.

Plan of Correction: We have purchased lots of carbon monoxide detectors for all of our centers. We were waiting until our painting was completed . We are installing them in all of our centers now. See attached invoice
