Comments:
The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 18, 2022 from approximately 12:45pm to 2:0pm. There were 19 children present, ranging in ages from one to four years, with three staff supervising. A total of five children?s files and four staff files were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-530-B
Description: Based on observation and interview, the vendor did not maintain a written hard copy record of daily attendance that documented the arrival and departure of each child in care as it occurred, for each grouping of children.
Evidence:
Staff # 3 showed the licensing inspector where children are signed in and out of the center. A member of management stated all the children are signed in and out off of one master attendance sheet.

Plan of Correction: Our Centers keep daily attendance records but effective 10.19.2022 every class room will keep their own daily attendance records for their classes

Standard #: 8VAC20-790-580-A
Description: Based on review of five children's records and interview, the vendor did not obtain documentation that each child had received the immunizations required by the State Board of Health before attending the center.

Evidence:
The record for Child #4 (first day of attendance 09/30/20 did not have documentation of immunizations. Staff #1 confirmed this information while the inspector was on site.

Plan of Correction: The child's mother was given a copy of the child's immunization record for school. She is bringing the copy back .We have informed all staff they must make a copy and never give the originals even thru the schools requested the original copy.

Description: Based on observation, the vendor did not ensure Based on observation, the vendor did not ensure cribs were provided for children from birth through 12 months of age .
Evidence:
1. Three infant were observed sleeping in playpens during the inspection.

Plan of Correction: We will ensure all infants sleep in designated cribs that meet current safety standards.
