Comments:
The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 18, 2022 from approximately 2:30pm to 2:45pm. There were 8 children present, ranging in ages from two to four years, with one staff supervising. A total of five children?s files and two staff files were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-530-B
Description: Based on observation and interview, the
vendor did not maintain a written hard copy
record of daily attendance that documented the
arrival and departure of each child in care as it
occurred, for each grouping of children.
Evidence:
Staff # 3 showed the licensing inspector where
children are signed in and out of the center. A
member of management stated all the children
are signed in and out off of one master
attendance sheet.

Plan of Correction: Our Centers keep daily attendance
records but effective 10.19.2022 every
class room will keep their own daily
attendance records for their classes

Standard #: 8VAC20-790-540-B
Description: Based on review of five children's records, the vendor did not ensure five children's records contained the required information.
Evidence:
The record for Child #1 did not the name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached.

Plan of Correction: The child's parents are providing another emergency contact numbers , address name and etc as requirements.

Standard #: 8VAC20-790-630-A
Description: Based on observation, the center did not
ensure all areas of the premises accessible to
children are free of obvious injury hazards,
including providing and maintaining sand or
other cushioning material under playground
equipment.
Evidence:
The door in the hallway leading into the kitchen
was broken and hanging off the hinges.

Plan of Correction: The door to our kitchen has been ordered its been on backorder at a local store due to the pandemic items has taken longer to receive . The door is due to be installed as soon as received, Children are never allowed in the kitchen.
