The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 4, 2022 from approximately 12:10pm until 2:00pm. There were zero children in attendance and a total of two staff members. A total of five children?s files and two staff files were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-790-540-B Description: Based on observation, the vendor did not ensure all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence:
The fence surrounding the outdoor playground had multiple areas of the fence that was broken and jagged pieces of wood were exposed.Plan of Correction: We will ask the family for the information and add it to the child's record.
Standard #: 8VAC20-790-550-2-f Description: Based on review of two staff records and interview, the vendor did not ensure documentation that background checks were conducted every five years.
Evidence:
The records for Staff #1 and Staff #2 contained satisfactory results of the Virginia Child Protective Services Central Registry dated 08/23/2017 and completed Sworn Statements dated 07/31/2017.Plan of Correction: We will request to update background checks.
Standard #: 8VAC20-790-630-A Description: Based on observation, the vendor did not ensure all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.

The fence surrounding the outdoor playground had multiple areas of the fence that was broken and jagged pieces of wood were exposed.Plan of Correction: We have contained the maintenance to have the fence fixed. We will follow back up and make the request again.
