Comments:
An unannounced code compliance inspection was initiated on 11/3/2022 and concluded on 11/3/2022 from 9:00 AM to 10:00 AM. There were 9 children present, ranging in ages from two to four, with 2 staff. Records were reviewed virtually. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, procedures and medication. A total of four children?s records and three staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22.1-289.031-A-4
Description: Based on record review, the center failed to ensure all staff have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him/her from caring for children under his/her supervision that was updated yearly.

Evidence:

1. The record for staff 1 has a sworn statement dated 9/17/21.
2. The record for staff 3 has a sworn statement dated 9/29/21.

Plan of Correction: Both staff 1 and staff 3 provided updated staff health reports on 11/4/22. In the future the director will develop a system to ensure staff health reports are updated yearly.
