An unannounced on-site renewal inspection was conducted on 09/07/2022. There were 3 children, with on staff person present during start of the inspection. The inspector observed the following: lunch and preparing for nap and nap. Total of 3 children?s records and 3 staff records, were reviewed. The inspector discussed the following with staff: answering the door as the inspector waiting for approximately 25 minutes, staff records, and parents

Standard #: 8VAC20-770-40-D-2 Description: Based on review of staff?s records, the center failed that all background checks were obtained.
Evidence:
1.Staff # 3 had a hire date of 7/25/2022. A return search of the central registry was not in the record. Staff # 1 did not provide documentation when requested.
2. Staff # 3 was hired on 7/25/2022 and there was no documentation of a Sworn Statement in the record on the day of the inspection.
3. Staff # 2 verified through interviews that they staff person was only working during the summer hours and the form was not completed.Plan of Correction: The staff person will be leaving the program and if they return they will have all required paperwork completed.
