A subsidy inspection was conducted on. A code inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Standard #: 8VAC20-790-560-A Description: Based on review of three staff?s records, the vendor failed to ensure that all staff all a TB test or screening at the time of employment and documentation shall be completed within the last 30 calendar days of employment.
Evidence:
Staff # 3 was working on the day of the inspection and did not have documentation of a TB test or screening. Staff documented start date was 7/25/2022.Plan of Correction: The staff person has left the program and in the future all staff will have the required information as required.
Standard #: 8VAC20-790-600-C Description: Based on review of three staff?s records, the vendor failed to ensure that orientation training was completed within seven days of the date of employment and prior to working with children alone.
Evidence:
1. Staff # 3 was hired on 7/25/2022 and was working alone on the day of the inspection did not have documentation of orientation within the required 7 days of employment and before working with children alone.Plan of Correction: The staff person has left the program and in the future all staff will have the required information as required.
