A code compliance inspection was completed on 11/30/22 from 2:35pm until 3:55pm. There were 9 children present with 3 adults supervising. One was a volunteer. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and 1 staff record was reviewed. Background check and general requirements were reviewed with the new director.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.031-B-5 Description: Based on observation, the center had an area on the playground that could cause an injury.
Evidence:
1. At the bottom of the rope ladder on the playground, there is an approximately 5 inches by 5 inches concrete support that is in the landing zone of the children.Plan of Correction: The director will work with church members to cover the support. Stall will provide extra supervision until fixed.
Standard #: 8VAC20-770-40-D-2 Description: Based on record review, the center failed to get the search of central registry within 30 days of employment.
Evidence:
1. Staff #1 was hired 08/31/2021 and there was no central registry results in the file.Plan of Correction: The director is trying to locate the central registry results as it has been completed. If not found, it will be repeated.
