Comments:

An unannounced Code Compliance inspection was conducted with the director on September 29, 2022 beginning at 11:48 AM and ending at 2:36 PM. The inspection was concluded on September 30, 2022. There were 15 children, ages 3 through 5 years old, in care during the inspection. The children were observed completing school work and eating lunch in the cafeteria. Five children's records were in compliance with the code requirements. Five staff records were reviewed. There were no medications on site to be reviewed. Facility policies and procedures were reviewed and in compliance. Standard #:	22.1-289.031-A-4
Description:	Based on a review of four staff records, the center failed to obtain documentation that all staff members are free from any disability which would prevent him from caring for children under his supervision. Evidence: Staff A, staff B, staff C, and staff D records were missing documentations of a current Staff Health Report. Staff A's record did not have documentation of a Staff Health Report. Staff B's most recent Staff Health report was dated 10/24/17. Staff C's most recent Staff Health report was dated 9/20/17. Staff D's most recent Staff Health report was dated 10/12/15.
Plan of Correction:	The center will request updated Staff Health Report forms from the staff and documentation will be placed in their record upon receipt.

Description:	Based on a review of five children's records, the center did not ensure that all children had documentation of immunizations prior to attending the center. Evidence. Child A did not have documentation of immunizations in their record.
Plan of Correction:	The center will request the missing documentation from the parents and update the child's record.
Standard #:	22.1-289.035-B-2
Description:	Based on a review of four staff records, the center failed to provide the department's representative documentation of fingerprinting background checks being completed for all staff members. Evidence: Documentation of a fingerprint background checks were requested for Staff A, staff C, and staff D by the Licensing Inspector on 9/29/2022. By the close of business 9/30/2022, no documentation was received from the director.
Plan of Correction:	The director did not have access to this documentation during the inspection. The director will obtain a copy of this documentation and place it a record that he has regular access to.

Description:	Based on a review of four staff records, the center failed to provide the department's representative documentation of completed sworn statement and central registry background checks being completed for all staff members. Evidence: Documentation of completed sworn statements and central registry background checks were requested for Staff A, staff C, and staff D by the Licensing Inspector on 9/29/2022. By the close of business 9/30/2022, no documentation was received from the director.
