A Code compliance inspection was initiated on 12/12/2022 and concluded on 12/12/2022. There were 17 children present with four staff supervising. The inspector reviewed compliance with the Code of Virginia. A total of five child records and seven staff records were reviewed. The inspection started at 9:50 am and concluded at 10:55 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Description: Based on record review the provider failed to ensure that All applicants for employment, employees, applicants to serve as volunteers, and volunteers shall undergo a background check in accordance with subsection B prior to employment or beginning to serve as a volunteer and every five years thereafter.
Evidence:
1. Staff # 3, 4, 6 and 7 have central registry reports that expired 9/22.Plan of Correction: The director will have everyone get their background checks repeated. The director will ensure that all background checks and sworn disclosure statements are repeated every five years.
