Comments:

A subsidy complaint inspection was initiated on 11/15/2022 and concluded on 01/05/2023. This complaint inspection was conducted with two local agencies. The inspector reviewed compliance in the areas of staffing and supervision and training. Information gathered during the investigation determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description:	Based on interviews, the center failed to report any suspected child abuse in accordance with the Code of Virginia. Evidence: On 11/15/2022 an anonymous report was made to the state hotline regarding an incident of suspected abuse. The incident date was 11/8/2022. On 11/16/2022 another incident was received by the local CPS. This incident date was 11/3/2022. None of the incidents were reported to the local department of social services by the center staff that are mandated reporters.

Description:	Based on record reviews and interviews, the vendor failed to inform the department?s representative as soon as practicable, but not more than two business days, of any injury to al child while under the vendor?s supervision when a referral is made for treatment from a medical profession. Evidence: An incident occurred on 11/8/22 in which a two year old sustained a head injury that required outside medical treatment. The Licensing Inspector received a report of this medical treatment from the provider on 11/29/22.

Description:	Based on interviews the center failed to ensure that forbidden actions such as physical punishment, including striking a child, roughly handling, or shaking a child were used. 1. On Nov. 3, 2022, a witness reported hearing staff #1 say, ?You don?t slap me.? Child #2 (age 3 years) was heard to be crying immediately after this. Staff #1 reported to parents that child #2 and another child spat on each other, and the child slapped child #2 in the face. Child #2 had a red mark on the right side of her check that resembled a handprint, which was larger than a child?s handprint. 2. On Nov. 8, 2022, child #1 (2 years old) was in the care of staff #1. Witness #1 stated that child #1 was ?slammed? down to a sitting position by staff #1 which resulted in child #1 head hitting the wall. Witness #2 stated that, staff #1 sat child #1 down forcefully which caused child #1 to hit head and cause an injury. Child #1 has stated that staff #1 ?pushed me?. Child #1 was taken to the local medical clinic by a family member at which time child #1 received two staples on the left, rear side of head. 3. On November 8, 2022, witness #2 stated that, staff #1 was holding child #3 (age 1 year) by the right arm and saw noticeable red marks. Witness #3 stated that, staff #1 grabbed child #3 by the upper arm and pulled child #3 down the hall to another classroom. Witness #3 described ?pulled? in a manner to where the tops of the child?s feet were dragging across the floor. Witness #3 also stated that child #3 had red marks on her arm and that child #3 was crying. 4. On November 15, 2022, witness #1 stated that staff #1 had set child #4 (22 months old) down ?hard? and child #4 hit head causing the child to cry. Staff #1 stated that, Staff #2 said for me to watch what I was doing, after sitting child #4 down.
