Conducted the annual religious exempt renewal inspection at 11:20am. Observed 50 children + 9 direct-care staff. Ratios and supervision were in compliance. Children were engaged in a variety of activities to include art, puzzles, blocks, science, outdoor active play, handwashing, and eating lunch. The facility was found to be clean and well maintained inside and outside. Classrooms were well supplied with toys and equipment for the children. Staff were actively engaged with the children and activities in all groups. An area of non-compliance was found with standards and laws reviewed. Standard #: 22.1-289.035-B-4 Description: Based on records reviewed, it was determined that 1 of 4 staff records reviewed did not include a required out-of-state background check within the 1st 30-days of employment. Evidence: Staff #13 ? Date of employment was 8/15/22 and to-date the out-of-state child abuse and neglect search has not been completed. Plan of Correction: The required documents will be submitted to the appropriate State within 10-days.

Evidence: Staff #13 ? Date of employment was 8/15/22 and to-date the VA child abuse and neglect search has not been completed. Plan of Correction: The Director will follow-up with the VA Central Registry this week to determine if either the e-request or paper request have been processed and determine when the results will be sent to her.
