Comments:
An annual monitoring inspection was initiated on 01/30/2023 and concluded on 01/30/2023. The inspector was on site at the center from 10:20am to 1:00pm. There were 129 children in care with 25 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playground, buses and all classrooms were inspected today. Ten children?s records and twelve employee records were reviewed during this inspection. Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were
documented on the violation notice issued to the vendor.

Standard #: 22.1-289.031-B-2
Description: Based on observation and an interview, the center did not ensure to implement the center?s established supervision procedures.

Evidence:

1. The center?s administrator stated that the supervision procedures established for the center are to ensure that all children are always within sight and sound supervision.
2. Upon entering the two year old classroom, there were 13 children within sight and sound supervision.
3. A two year old teacher stated that there were 14 children in care.
4. Approximately 3-5 minutes after the inspector entered the two year old classroom it was discovered that a two year old child was playing in the open closet in the classroom, out of sight supervision.

Description: Based on record review, the center did not ensure three of twelve staff records contained documentation of criminal history record results, sex offender registry check results and a child abuse and neglect search request from any state in which the individual has resided in the past five years.

Evidence:
1. The record of staff #5 (employment date: 2/7/2022) did not have documentation of out of state sex offender registry check results from any state in which the individual has resided in the past five years.
2. The record of staff #7 (start date: 6/6/2022) did not have documentation of sex offender registry check results from any state in which the individual has resided in the past five years.
3. The record of staff #10 (start date: 7/1/2022) did not have documentation of criminal history record results, sex offender registry check results and a child abuse and neglect search request from any state in which the individual has resided in the past five years.
