Comments:
An annual monitoring inspection was initiated on 02/1/2023 and concluded on 02/1/2023. The inspector was on site at the center from 12:05am to 2:00pm. There were 43 children in care with 9 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playground and all classrooms were inspected today. Five children?s records and five employee records were reviewed during this inspection. Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Violations:
Standard #: 22.1-289.031-A-5-a
Description: Based on observations and review of documentation, the center did not ensure compliance with the terms of the religious exemption issued by the Department of Education.
Evidence:
The letter of exemption states that no more than five children under the age of two and one half are allowed at any one time. During the 02/01/2023 inspection, seven infants (six weeks to 16 months) were being cared for by two staff members in the infant Room and eight Toddlers (17 months to 23 moths) were being cared for by two staff members in the toddler Room. The infant and toddler rooms do not have classroom doors that lead directly to the exterior.

Plan of Correction: The infant classroom will operate with only 5 infants, which will keep our preschool in compliance with our current license issued by VDOE. We have since closed down our Toddler classroom until we can address the building modification that needs to take place to bring our building up to code.

Standard #: 22.1-289.031-B-3
Description: Based on record review and an interview, the center did not ensure to have at least one person on staff that has been trained to conduct daily simple health screenings of children.
Evidence:
During the inspection of five staff records and an interview with the center representative, it was discovered there were no staff that received training in daily health screenings within the past three years. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings. Daily health screenings should be completed every three years.

Plan of Correction: Two staff members have completed daily health screening trainings.

Description: Based on record review, the vendor did not ensure one of five staff records contained documentation of fingerprint based national criminal history search results prior to employment.
Evidence:
1. The record of staff #4 (employment date: 9/6/2022) did not have documentation of fingerprint based national criminal history search results.

Plan of Correction: Staff #4 has completed an updated fingerprinting.

Description: Based on record review, the center did not ensure one of five staff records contained documentation of criminal history record results and sex offender registry check results from any state in which the individual has resided in the past five years.

Evidence:
1. The record of staff #1 (employment date: 2/1/2023) did not have documentation of out of state sex offender registry check results and criminal history results.
2. The record of staff #1 indicated that the staff member has lived outside of Virginia within the last 5 years.

Plan of Correction: Documentation has been placed in staff #1 employee folder, for out of state (Ohio) sex offender registry check and criminal history results.

Standard #: 22.1-289.058
Description: Based on observation and an interview, the center did not ensure that the building serving preschool aged children was equipped with at least one carbon monoxide detector.

Evidence:
The licensing inspector did not see a carbon monoxide detector installed in the center. The center?s administrator stated that a carbon monoxide detector is not currently installed in the building.

Plan of Correction: A carbon monoxide detector has been installed in the preschool building.

Description: Based on record review, the provider did not ensure 4 of 5 staff records had documentation of central registry results within 30 days of employment.
Evidence:
1. The record of staff #2 (employment date: 8/29/22) did not have documentation of central registry results.
2. The record of staff #3 (employment date: 9/22/22) did not have documentation of central registry results.
3. The record of staff #4 (employment date: 9/6/22) did not have documentation of central registry results.
4. The record of staff #5 (employment date: 9/15/22) did not have documentation of central registry results.

Plan of Correction: All staff members have been entered into the Virginia Department of Social Services Central Database, order confirmation number: 11105
