Licensing Inspector conducted an unannounced CODE Monitoring Inspection on February 2, 2023. There were 44 children ages7 months to 4 years old present under the direct care of 9 staff. The Licensing Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-2 Description: Based on record review, the Vendor failed to ensure each staff person completes a fingerprint-based criminal history background check prior to 1st day of employment
EVIDENCE:
Licensing Inspector reviewed 5 staff records. Record for Staff #4 did not include a fingerprint-based criminal history determination letter. Date of hire for Staff #4 is 3/14/2022.Plan of Correction: Staff will schedule and complete a fingerprint background check. Vendor will submit proof of correction to the Licensing Inspector within 10 days.
