An annual monitoring inspection was initiated on 02/13/2023 and concluded on 02/13/2023. The inspector was on site at the center from 10:15am to 12:40pm. There were 12 children in care with 2 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playground and all classrooms were inspected today. Five children?s records and six employee records were reviewed during this inspection. Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Standard #: 22.1-289.031-A-4 Description: Based on record review, the center did not ensure each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
Evidence:
1. The record of staff #1 (employment date: 7/5/22) did not have documentation showing that they have been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.

Standard #: 22.1-289.031-B-3 Description: Based on record review and an interview, the center did not ensure to have at least one person on staff that has been trained to conduct daily simple health screenings of children.
Evidence:
During the inspection of six staff records and an interview with the center representative, it was discovered there were no staff that received training in daily health screenings within the past three years. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings.
