Comments:
Licensing Inspector conducted an Announced Initial Subsidy Inspection on February 15, 2023 There were 37 children ages 4 months to 4 years old present under the direct care of 8 staff. The Licensing Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-550-1
Description: Based on record review, the Vendor failed to have date of employment for all staff members.

EVIDENCE:
Licensing Inspector reviewed 4 staff records.

Records for Staff #2 and Staff #4 did not include date of hire.

Plan of Correction: Director will document age verification for each staff.

Standard #: 8VAC20-790-560-A
Description: Based on record review, the Vendor failed to ensure each staff member obtains a tuberculosis screening at the time of employment.

EVIDENCE:
Licensing Inspector reviewed 4 staff records.

Record for Staff #3 (date of hire 12/30/2022) did not include documentation of a negative tuberculosis screening.

Plan of Correction: Staff member has scheduled an appointment to complete TB screening

Standard #: 8VAC20-790-600-I
Description: Based on inspection, the Vendor failed to have at least one staff on duty who has obtained within the last three years instruction in performing a daily health observation of children.

EVIDENCE:
There was not at least 1 staff person present during the inspection who has completed daily health observations of children.

Plan of Correction: Each staff member will receive daily health observation training.

Standard #: 8VAC20-790-640-A
Description: Based on inspection, the Vendor failed to keep hazardous substances in a locked place using a safe locking method that prevents access by children.

EVIDENCE:
Hazardous cleaning agents were not stored in a locked location. Licensing Inspector observed unlock cleaning agents in 3 out of 4 classrooms that were inspected.

Plan of Correction: All hazardous cleaning agents will be stored in a locked cabinet. Corrected during inspecttion

Description: Based on inspection, the Vendor failed to develop and implement a written policy and procedure that describes how the Vendor will ensure that each group of children received care by consistent staff or team of staff members.

EVIDENCE:
Plan of continuity policy and procedures have not been developed.

Plan of Correction: Director will develop a plan of continuity for the center.

Description: Based on observations, the Vendor failed to ensure staff wash their hands with soap and running water before and after changing a diaper.

EVIDENCE:
Staff member in the infant classroom did not wash her hands after changing an infants' diaper.

Plan of Correction: Staff will wash their hands before and after changing diapers. Corrected during inspection

Standard #: 8VAC20-790-760-B-6
Description: Based on observations, the Vendor failed to use a foot-operated container for disposable diapers.

EVIDENCE:
In the infant classroom, staff member was observed using her hand to open the foot operated trash can for disposable diapers. The trash can had a child safety strap attached to the can to prevent children from opening the can.

Plan of Correction: The child safety strap will be removed from the foot operated trash can. Corrected during inspection
