Discussed keeping allergy list confidential, electric outlets , hazordous substances and Update Virginia Preservice Training.

Comments:

A subsidy health and safety (SHSI) inspection was completed on February 14, 2023. There were 66 children present 13 staff and additional staff present to assist. During the inspection children were observed eating, dancing, preparing for activities after nap , PE activity outside, playing outside and transition to outside activity. Five children records were reviewed and nine staff records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable requirements and violations were documented on the violation notice issued to the program.

Description:	Based on staff interview and review of record, the vendor did not enure a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: Staff confirmed that there was not a written record of daily attendance that document departure of each child as it occured in the Preschool room.

Description:	Based on staff interview, the vendor did not enure that there is documentation of Orientation that includes date completed total hours of a session , and names of the trainer and any sponsoring organizaiton. Description:	Based on record review and staff interview, the licensee did not ensure documentation of a tuberculosis screening was submitted at the time of employment and completed within the last 30 days of the date of employment . Evidence: Staff #1 hire date 08/29/2022, Staff #3 hire date 07/08/2022 and Staff #7 hire date 6/6/2022, did not have documentation TB test results. Staff #4 hire date 10/18/22 TB test documentation did not have results of the TB test. Staff # 8 hire date 01/16/2023 TB test was completed 05/30/2022 which is more than 30days prior of employment.

Evidence: Outside on the play ground there was a wooden climbing arch connected to the playground composite. One of the wood slats was splintering which could snag a child or get entangled on a shoelace.
