Comments:
An annual monitoring inspection was initiated on 04/19/2023 and concluded on 04/19/2023. The inspector was on site at the center from approximately 11:15am to 2:15pm. There were 26 children in care with 6 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playground and classrooms were inspected today. Five children?s records and five employee records were reviewed during this inspection. Information gathered during the inspection determined areas of noncompliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Description: Based on observation, the center did not ensure that all children in the center are in compliance with the provisions of ? 32.1-46
regarding the immunization of children against certain diseases.
Evidence:
1. The record of child #1 (enrollment date: 10/14/2022) did not have documentation of immunizations.
2. The record of child #2 (enrollment date: 3/17/2023) did not have documentation of immunizations.
3. The record of child #3 (enrollment date: 1/17/2023) had documentation of immunizations dated 2/27/2023.

Standard #: 22.1-289.031-A-9
Description: Based on observation, the child day center was not in compliance with safe sleep guidelines recommended by the American Academy of Pediatrics.
Evidence:
1. The inspector observed an infant sleeping in a crib with a blanket underneath the infants head, one blanket on top of the infant and another blanket and cloth bib on the side of the infant.
2. The inspector observed two other infants sleeping in a crib with a blanket covering the each of them.
3. The American Academy of Pediatrics state that infant sleep should be avoided with ?loose blankets, pillows, stuffed toys, bumpers, and other soft items out of the sleep space.?

Description: Based on observation, the center did not ensure to implement the center?s established procedures regarding hazardous substances.
Evidence:
1. The center requires that all cleaners and hazardous substances be locked.
2. There were several containers of cleaners located in the center?s closet and kitchen. Both the closet and the kitchen were unlocked.

Description: Based on record review and an interview, the center did not ensure one of five staff records contained documentation of criminal history record results, sex offender registry check results and a child abuse and neglect search request from any state in which the individual has resided in the past five years.
Evidence:
1. The record of staff #2 (employment date: 9/19/2022) did not have documentation of out of state criminal history record results, sex offender registry check results and a child abuse and neglect search request.
2. The record of staff #2 indicated that the staff member has lived outside of Virginia within the last 5 years. Staff #2 confirmed that she has lived outside of Virginia within the last five years.
