Discussed ensuring that the facility is using the most up-to-date background check forms by going to https://www.childcare.virginia.gov/ to get the most recent forms.

A code compliance inspection of religious exempt child day centers was conducted between 9:30 and 11:45 AM in conjunction with a subsidy health and safety inspection. The sample size was 19 staff records. A violation is noted on the attached violation notice.

Standard #: 22.1-289.035-B-4 Description: Based on a review of 19 staff records, it was determined that the vendor did not ensure that all staff who had lived out of state within the past five years had a criminal record check and a central registry check on file.
Evidence:
1. Staff #9 indicated on her sworn statement that she had lived out of state in the past five years. There was not a criminal history check or a search of the central registry on file.
2. Staff #13 indicated her sworn statement that she had lived out of state in the past five years. There was not a criminal history check or a search of the central registry on file.
3. Staff #16 indicated her sworn statement that she had lived out of state in two different states in the past five years. Both states were considered "NFF" states, so criminal history checks were not required. However, there were not central registry findings from either state.Plan of Correction: Facility will request appropriate background checks and when results are received, they will be placed in staff member's records.
Any new staff members hired, who have lived out of state in the past five years, will have appropriate background checks obtained prior to employment.
