8VAC20-790 Subsidy Program Vendor Requirements for Child Day Centers

Comments:
A subsidy health and safety inspection was conducted from10:15 - 11:45 AM. Children were observed participating in free play indoors and outdoors and eating lunch. The sample size was 19 staff and five children's records.

Violations:
Standard #: 8VAC20-790-560-A
Description: Based on review of 19 staff records, it was determined that the vendor did not ensure that all staff had tuberculosis (Tb) screenings on file prior to employment that has been conducted within the past 30 days.

Evidence:
1. The Tb screening for staff #8 (date of hire 11/28/22) was dated 8/12/21; which was more than 30 days before hire.
2. The Tb screening for staff #17 (date of hire 1/26/23) was dated 1/27/22; which was after her date of employment.
3. The Tb screening for staff #19 (date of hire 3/23/23) was dated 1/30/23; which was more than 30 days before hire.

Plan of Correction: Facility director will ensure that all staff member have appropriately timed Tb screenings on file prior to beginning employment.

Description: Based on review of 19 staff records, it was determined that the vendor did not ensure that tuberculosis (Tb) screenings were updated every two years from the date of the last screening.

Evidence:
The last Tb screening for staff #10 was dated 3/22/21, and was therefore over two years old.

Plan of Correction: Facility will ensure that all Tb screenings are updated every two years.

Description: Based on review of 19 staff records, it was determined that the vendor did not ensure that all staff completed the Virginia Preservice Training for Child Care Staff within 90 days of employment.

Evidence:
The following staff did not have evidence of completing preservice training in their files:
- staff #1 (date of hire 10/31/22)
- staff #2 (date of hire 5/24/22)
- staff #4 (date of hire 8/8/22)
- staff #5 (date of hire 5/5/21)
- staff #6 (date of hire 12/1/22)

Description: Based on observation, it was determined that the vendor did not ensure that diapers were disposed of in a leak-proof that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Evidence:
The diaper disposal system being used throughout the facility is a diaper genie that requires staff to close the system with their hand.

Plan of Correction: Facility will obtain new diaper disposal systems to use in the classrooms.
