Technical Assistance:

The center is not currently in compliance with Virginia Code 22.1-289.049 which requires that upon enrollment of a child in a regulated child day program, such child day program shall require information from the person enrolling the child regarding previous child day care and schools attended by the child. The regulated child day program shall also require that the person enrolling the child present the regulated child day program with the proof of the child's identity and age. One of five child records reviewed did not contain documentation the the child's proof of age and identity was verified by the center.

Comments:

An unannounced Code Compliance inspection was conducted with the Director on 4/25/2023 from 9:30am through 10:55am. There were 37 children present supervised by five staff. The children were observed playing outside and participating in circle time. Five child records were reviewed for documentation of immunizations and verification of proof of age and identity. Five staff records were reviewed for background checks, annual health statements, and training. The program has multiple staff with current certification in CPR and First Aid and one staff with Medication Administration Training. Areas of non-compliance are identified in the Violation Notice. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 5/11/2023. Description:	Based on record review, the center did not obtain health statements for all staff signed by a physician or physician assistant indicating that the staff is free from any disability which would prevent them from caring for children under their supervision. Description:	Based on observation and staff interview, the center did not ensure that required written information was made available to the parents or guardians of children in the center or made available to the general public. Evidence: Information regarding physical facilities, food services, health requirements for staff and public liability insurance was not posted and accessible to the general public at the time of inspection.

Description:	Based on record review and staff interview, the center did not ensure that daily simple health screening are conducted by a person trained to perform such screenings. Evidence: Records for all five staff present at the time of inspection were reviewed and no documentation was maintained that any staff have Daily Health Observation training.

Description:	Based on record review, the center did not obtain the results of a criminal history record check prior to employment or request a search of the child abuse and neglect central registry within 30 days of hire from all states staff have resided within the preceding five years. Evidence: Staff #2 has resided in the state of Washington within the last five years and did not have the results of a Washington criminal history record check or documentation that a search request of the Washington child abuse and neglect central registry was made.

Description:	Based on record review, the center did not ensure that all staff submit a Sworn Disclosure Statement prior to employment or the results of a Virginia Child Abuse and Neglect Central Registry search within 30 days of employment or commencement of service.
