Standard #: 22.1-289.031-A-6 Description: Based on observation and interview it was determined that the required aspects of the child day center's operations were not provided in a written statement to the parent and made available to the general public. Evidence: The required written statement in regards to the center's physical facilities, enrollment capacity, food services, health requirements for the staff, and public liability insurance was not provided to parents/guardians or made available to the general public. Plan of Correction: Administration will post the required information in a conspicuous place to the general public and provide the required information to parents/guardians.
