An unannounced "Code of Virginia Section 63.2-1716" compliance monitoring inspection was conducted on May 11, 2023 at 11am, and observed 63 children, ages 2-5, were under the care and supervision of 13 staff. Children were observed participating in a lessons, playing at centers, playing outside, having playtime in the playroom. Center documents, 5 children?s records and 8 staff records were reviewed. Standard #: 22.1-289.035-A Description: Based on review, the facility did not ensure that background checks were repeated every 5 years.
Evidence: reviewed 8 staff files and found that Staff 3 and Staff 4 did not have up to date sworn statements of disclosure. Staff 3's last statement was signed on 8/10/17 and Staff 4's last statement was signed on 8/19/14.Plan of Correction: Not available online. Contact Inspector for more information.
