An unannounced VA Code Compliance monitoring inspection was conducted from 11:45 am - 12:45 pm. There were 20 children in care being supervised by 3 staff. The children were observed eating lunch.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.031-B-3 Description: Based on review of three staff records and interviews it was determined that there was not staff on site who had received daily health observation training. Evidence: The records for staff A, B, and C did not contain documentation of daily health observation training. It was reported by administration that staff had not received daily health observation training. Plan of Correction: Staff will receive daily health observation training.
Standard #: 22.1-289.031-B-6 Description: Based on review of three staff records and interviews it was determined that not all records contained documentation of child abuse and neglect training. Evidence: The record for staff A (start date: 09/06/2022) and staff B (start date: 08/15/2015) did not have documentation of receiving child abuse and neglect training. Plan of Correction: Staff will obtain child abuse and neglect training.
Standard #: 22.1-289.035-A Description: Based on review of three staff records it was determined that not all background checks were repeated every five years. Evidence: The most recent central registry check on file for Staff A is dated 10/23/2015. The most recent Sworn Statement or Affirmation form on file for Staff C is dated 10/18/2016. Plan of Correction: A repeat central registry check will be obtained for Staff B. Staff C will complete a sworn statement or affirmation form.
Standard #: 22.1-289.035-B-2 Description: Based on review of three staff records it was determined that not all staff records contained documentation of a determination letter based on a criminal fingerprint check before their first day of employment. Evidence: The record for staff A (start date: 09/06/22) did not have documentation of a determination letter based on a criminal fingerprint check. Plan of Correction: Staff A will obtain a determination letter based on a criminal fingerprint check.
