An investigation was initiated on 5/15/23 in response to a complaint received on 5/9/23 and 5/14/23 regarding allegations in the areas of building maintenance, ratios and behavior guidance. An on-site inspection was conducted on 5/23/23 that included classroom observation and interviews with staff. Documentation and records related to the complaint were also reviewed. Based on the information gathered during the complaint investigation, the allegation complaint regarding building maintenance is determined to be valid.

Standard #: 8VAC20-790-630-A Complaint related: Yes Description: Based on observation and interviews the vendor did not ensure that areas and equipment of the center shall be maintained in a safe condition.
Evidence:
1. There was an unlocked deep freezer in a room where children nap. The inspector observed a preschool age child within 4 feet of the freezer. The Director confirmed that the freezer did not lock.
2. In the Older 1's classroom there was a 4" hole on the base of the bathroom wall and there was exposed, crumbling drywall. Children have access to this area. Staff #4 confirmed that there was a hole in the wall that was accessible to children.
3. In the Older 1's classroom there was a wood door frame with cracks and peeling paint. Staff #4 confirmed that there were cracks and peeling paint that was accessible to children.Plan of Correction: A safety latched for the deep freezer has been installed. The freezer is unplugged and will be removed from the area. A board has been put in place to cover /repair the hole in the wall of the older 1's bathroom.
Standard #: 8VAC20-790-760-B-6 Complaint related: No Description: Based on observation and interviews, it was determined that the vendor did not ensure that disposable diapers were disposed of in a storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system.
Evidence:
In the Older 1's classroom there was a diaper genie being used. The hands free, foot operated step mechanism was not working. Staff #4 confirmed that they must open the lid with their hands because the foot mechanism did not work.Plan of Correction: Not available online. Contact Inspector for more information.
