Licensing Inspector conducted an unannounced CODE Compliance Inspection on 07/31/2023 There were 16 children ages 20 months to 4 years old present under the direct care of 3 staff. The Licensing Inspector reviewed 4 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program Due to number of violations related to background checks, an Intensive Plan of Correction (IPOC) will be required. The inspector arrived at 10 AM and departed at 11:30 AM. Description:	REPEAT VIOLATION Based on record review, the Center failed to ensure staff members supervising children have a completed staff health report. EVIDENCE: 1. Staff #3 was observed supervising children during the inspection. There was no documentation of a completed staff health report. 2. Staff #5 was observed supervising children during the inspection. There was no documentation of a completed staff health report.

Description:	REPEAT VIOLATION SYSTEMIC DEFICIENCY Based on record review and interview with staff, the Center failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia. Evidence: Staff #3 was observed supervising children during the inspection. There was no employment eligibility letter available for review during the inspection. Staff #5 stated the hire date for Staff #3 was in May 2023.

Description:	SYSTEMIC DEFICIENCY Based on review of five staff?s records, the center failed to ensure that all required out of state background checks were obtained. Evidence: 1. Staff #1 indicated on the sworn statement that they have lived outside of the state of Virginia in the past five years. An out of state criminal history records check and an out of state child abuse and neglect search was not available for review. Staff #5 stated that the hire date for Staff #1 was in March 2023. 2. Staff #4 indicated on the sworn statement that they have lived outside of the state of Virginia in the past five years. An out of state criminal history records check and an out of state child abuse and neglect search was not available for review. Staff #5 stated that the hire date for Staff #4 was in February 2023. 3. Staff #5 stated through interviews that they have lived in two different states outside of Virginia in the past five years. An out of state criminal history records check and an out of state child abuse and neglect search was not available for review for both states. Staff #5 stated that their hire date was in February 2023.
