Technical Assistance: Code of Virginia (22.1-289.049): The regulated child day program shall require that the person enrolling the child present the regulated child day program with the proof of the child's identity and age.

Comments:
An unannounced Code Compliance inspection was conducted with the director on 8/23/23 from 9:25am to 11:30am. There were 48 children present, ranging in age from two- to five-years-old, supervised by 10 staff. Children were observed playing in structured centers, working individually with staff, and playing on the playground. The required postings, classrooms, and playground were observed. Five child records and five staff records were reviewed for required documentation. The program met the requirement for staff on-site with current CPR/First Aid training. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 9/5/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 22.1-289.031-B-3
Description: Based on interview, the program did not ensure that a daily simple health screening and exclusion of sick children was conducted by a person trained to perform such screenings. Evidence: Staff reported that they were not aware of the training and did not believe anyone currently had it.

Plan of Correction: The director of the child care program has taken the course on the daily health check that is required to perform daily health check screenings. The director has also requested 5 teachers also take the daily health screening course online.

Standard #: 22.1-289.031-B-4
Description: Based on review of five child records and interview, the program did not ensure that all children were in compliance with immunization provisions. Evidence: The record for Child E, with a start date of 7/24/23, did not contain documentation of immunizations.

Plan of Correction: Parent brought immunization records in the following day due to directors persistence in getting the child's records up to date.

Standard #: 22.1-289.035-B-2
Description: Based on review of five staff records, the program did not obtain the results of a fingerprint-based national criminal history record check prior to employment for each staff. Evidence: The fingerprints on record for Staff #2, with an employment date of 8/16/22, were dated 8/17/22. The fingerprints on record for Staff #4, with an employment date of 5/3/22, were dated 5/10/22.

Plan of Correction: Office Staff will correct this violation by interviewing and fingerprinting staff in the same day. Furthermore, Warrenton Baptist Tiny Tots will not offer the position until after the results of the fingerprint record check is returned to us and the applicant is in good standing. It is at that time that office staff will give additional paperwork and offer employment.

Description: Based on review of five staff records, the program did not obtain documentation of a completed sworn disclosure statement prior to employment, or the results of a search of the central registry within 30 days of employment, for each staff. Evidence: 1. The sworn disclosure on record for Staff #4, with an employment date of 5/3/22, was dated 5/6/22. 2. The central registry results on record for Staff #2, with an employment date of 8/16/22, were dated 9/30/22. The central registry results on record for Staff #4, with an employment date of 5/3/22, were dated 6/23/22.

Plan of Correction: Office Staff will correct this violation by including the sworn disclosure statement on the same day of the applicant's interview. With this policy, the applicant will not be offered employment until after the sworn statement is turned in and returned with good standing. It would be at that time, Warrenton Baptist Tiny Tots will then offer employment.
